UP Baguio - WiFi Connection Request Form (Students)
PURPOSE:  This form is used by the UPB Computer/LAN Office (C/LAN) for collecting data and information to be used for the provisioning of campus WiFi access for UPB students.  Students can register upto two (2) WiFi devices for use.  Registered devices can be changed at any time.

INSTRUCTIONS: Please fill-in as completely and accurately as possible to avoid registration delays and problems.
