STREET OUTREACH REFERRAL FORM
HOPES Center's street outreach team is out several nights per week to look for and assist people who are sleep in places not meant for human habitation at night. Places not meant for human habitation include places like benches, parks, wooded areas, pavilions, bus shelters, abandoned buildings, vehicles, under bridges, and places like that. The team does not go onto people's private property (e.g. people's garages , dive ways, or backyards) and does not enter private buildings (such as apartment buildings or occupied houses) to look for people.

If you are living outside, you can use this form to help facilitate contact with the street outreach team at night. After submitting the form, the team will look for you in the location you provide the next time it is out on the streets. The team carries supplies that may be useful to you while you are staying outside and can help connect you to services to end you current homeless situation.

Street outreach is a confidential services. The team will not tell anyone where you are staying and will not share any conversations you have with the team.
