Creekside Middle School PTO Membership/Volunteer Form 2018-2019
Your time, talent and enthusiasm are the heart of the Creekside Middle School PTO! Please take a look at the volunteer committee opportunities and sign up for as many as you like. All parents and guardians are welcome and encouraged to participate. Share your expertise or learn something new and make a difference.  Volunteering is fun and easy! No prior training or experience is necessary.  

Your Creekside Parent Teacher Organization provides funding and volunteers for Creekside Middle School academics, sports and arts.  Parents are encouraged to volunteer on one of the committees below.  You will receive further details and instructions from the leader of your volunteer committee within the upcoming weeks.  

Your donations and active participation are critical to the success of the PTO. Please make your tax-deductible donation to support programs provided by the PTO.

Voluntary Tax Deductible Donation *
We are asking each family to make a donation.  So if you haven’t already made your donation, please take a moment to do it now.  Your donations help all children of Creekside Middle School. There are four donation levels:  $200.00 (Diamond Sponsor) will receive a stylish Carmel Schools Upside Down Umbrella.  $150.00 (Platinum Sponsor) will receive a Stainless Steel Creekside Tumbler w/ Lid.  $100.00 (Gold Sponsor) will receive a Creekside USB Charger that attaches to their child’s backpack.   $75.00 (Silver Sponsor) will receive a Creekside Car Decal to show their Wildcat Pride!  Many of the school programs and activities for the children will not be possible without your generous donation.

Fundraising Committee
If your time is quite limited, we'll contact you to assist with various fundraising projects through the school year.  Chair: Geri Imler
Scholastic Book Fair
Committee coordinates the sale of books to raise money for media center supplementary books through Scholastic Book Fair which will be held in November. Chair: Jessica Brand
Book Store
Parents volunteer on a rotating basis during lunch. Chair: Beth Kurlander
Cultural Fair - March 2019
Held every other year,  this event celebrates the multicultural community at Creekside. Date TBD, usually in February or March.
Eighth Grade Celebration
Help plan and chaperon end-of-year celebration. Party will be held on TBD, at the school.  Chair: Cherie Castle
Hospitality
Coordination of food for certain staff meetings, year end staff breakfast, bus driver breakfast and other staff recognition. Coordinate and deliver baked goods for staff throughout the year. Chair: Kim Berebitsky
Media Center
Assist in media center on a rotating basis.
