I, as parent/legal guardian of the above-named athlete, hereby grant permission for them to participate in the Bulldog Baseball Camp and acknowledge the fact that they are physically able to participate in camp activities. We understand that the Bulldog Baseball Camp does not provide medical insurance covering injuries of any nature incurred during the 2024 Bulldog Baseball Camp. All participants should be covered by their own insurance.
