Oak Hill Middle School Absence Reporting 2024-2025
Please use this form to report your student's absence from school. Absences include full-day absences, late arrivals, and early dismissals. Please note that attendance in school is vital for good performance. To help your student achieve all they can, please limit absences, late arrivals, and early dismissals to illness, urgent medical appointments, or other unavoidable appointments.

This form is for parent or guardian use only. This request MUST be verified by a parent or guardian email address that is currently in Aspen, connected to your student's portal. Any student who misrepresents themselves as their guardian by filling out this form are subject to suspension for forgery.

Please be aware: Vacations, travel, and family trips are NOT considered excused absences.
