REQUIREMENTS AND RESPONSIBILITIES

All volunteers are required to practice kitchen safety and hygiene standards set forth by The Kidz Kitchen, including proper hand washing techniques, wearing closed-toe shoes (no sandals or crocs), tying back long hair, wearing short sleeves or sleeves that can be easily rolled up.  Please check the box to indicate that you are in agreement with this requirement.

All volunteers who work with children are required to participate in "Safe Church Training," a series of informational videos provided by the Episcopal Church. You will need to watch the videos prior to volunteering in The Kidz Kitchen. Please check the box to indicate that you are in agreement with this requirement.

Dear Applicant/Parent/Guardian, 
The Kidz Kitchen of NJ is requesting your permission to use your photos/videos for advertising and social media purposes. We plan to use photos/videos on our social media page (Facebook, Instagram), website, marketing materials to promote our program and highlight the fun and engaging activities we offer. We want to assure you that we will take all necessary steps to protect your  privacy. We will only use the photograph in the ways described above, and we will not sell or share the photograph with any third parties.

Hold Harmless Agreement, Waiver and Release:

In consideration of being permitted by The Kidz Kitchen of NJ and Grace-St. Paul's Church, to participate in its cooking classes, I, the participant or undersigned parent/legal guardian of the participant named below, do hereby waive, release, and discharge any and all claims for damages or personal injury, death, or property damage which I or the participant may have or which may hereafter accrue as a result of participation in said activity. I understand and acknowledge the following:

● The activities may involve the use of kitchen equipment, utensils, and food items that can cause injury if not used properly.

● There are inherent risks associated with cooking, such as cuts, burns, and allergic reactions, which cannot be completely eliminated.

● The Kidz Kitchen will take all reasonable precautions to ensure the safety of participants, but accidents can and do happen.

● I (participant)/my child (if under 18) is participating in this activity voluntarily and I assume all risks associated with my/their participation. By signing this form, I agree to the following:

● I will hold harmless and defend The Kidz Kitchen, its directors, officers, employees, volunteers, agents and Grace- St. Paul's Church, from any and all claims, demands, losses, liabilities, costs, and expenses arising out of or in any way connected with myslf/my child’s self-participation.

● I will provide The Kidz Kitchen with any and all information regarding myself/my child's medical conditions, allergies, and dietary restrictions.

● I will ensure that myself/my child follows all safety rules and instructions provided by The Kidz Kitchen staff.

● I will remove myself/my child from the program if I/they become disruptive or pose a safety risk to themselves or others.
