This completed form is due by Sunday, October 4th. Without this form, your entry/entries will not be complete. NO LATE ENTRIES WILL BE ACCEPTED.

The undersigned agrees to indemnify the Hawai'i Forest Industry Association, Hawaii's Woodshow, and Hawaii Opera Theatre/Hawaii Opera Plaza from any damages that may occur to any piece(s) of his or her making during the course of the above mentioned exhibition or transit of the piece to the exhibition and submits said piece(s) at his or her own risk.

Entry Fees. These may be submitted via Paypal on the website or via mail with a check made out to HFIA. Fees for Entry #1 for new and renewing members include a one-year HFIA membership. Current memberships must be valid through October 2020.
Entry #1 for non-member or renewing Individual member - $110.00
Entry #1 for non-member or renewing Business member - $185.00
Entry #1 for non-member or renewing Corporation member - $360.00
Entry #1 for current member - $60.00
