Objective:
To recognize a member of the Muslim healthcare community who has been an instrumental and exemplary mentor for one or more peers.
Award:
The award will consist of a plaque and a token of appreciation in recognition of their time and commitment to providing quality and effective mentorship.
Application Process:
Any person may nominate a member of the Muslim healthcare community for this award by completing the nomination form on page 2 of this application. Self-nominations are not accepted. There are no limits to the number of times a person may be nominated.
Assessment of Application:
Nominations will be reviewed by two independent members of the MMAC Board of Directors or their designate, at least one of whom will be part of the MMAC’s mentorship subcommittee. The nominees will be assessed based on the method, quality, duration and impact of the mentorship provided. Successful candidates will have embodied one or more of the following characteristics or activities:availability & approachability, strong communication skills, honesty and objectivity, sincere interest in the mentee’s success, compassion, leadership, intellectual support, assistance with networking and/or career advancement and personal support.

Submitting the Application:
Nominations must be submitted by Sunday April 30th, 2023 at 11:59pm EST. Late or incomplete applications will be discarded. Terms subject to change without prior notice.
