NE-9, Youth Ministry Registration, 2023-2024
***This form is to register your 7-12th graders for youth ministry in our Family of Parishes for 2023-2024. Please complete and submit ONE FORM PER FAMILY.***

ACTIVITY DESCRIPTION
This registration will cover your High School or Jr. High teen's participation in weekly youth ministry at our parish locations. Jr. High is open to 7-8th grade. High School is open to 9-12th grade. Activities will include games, sports, small group ministry, prayer, Mass, faith formation, etc., as well as occasional off-site activities. This will also cover your teen's participation as a peer leader, if applicable. Open to all teens in our Family of Parishes - public school, Catholic school, and home-school. Feel free to contact Jeffrey with any schedule conflicts. 

MEETING DAYS/TIMES: 
High School: 
Sunday, 7:00-8:30 PM (St. Mary)
Peer Leaders will also have some additional trainings. 

Jr. High: 
Monday, 3:30-5:00 PM (St. Henry, PAC, pending interest)
Monday, 6:00-7:30 PM (St. Henry, PAC)
Wednesday, 5:30-7:00 PM (St. Mary, Parish Center)

DATES:
September 2023 through May 2024; (June-August 2024 where applicable).  

LOCATION:  
Monday (SH):  Parish Activities Center, 6660 Springboro Pike, Dayton, OH 45449
Wednesday & Sunday (SM):  9579 Yankee Rd, Springboro, OH, 45066 
Note: Our Lady of Good Hope, St. Henry and Bishop Leibold School locations may also be used for sacrament prep activities, or in the event a certain campus is unavailable.

CALENDAR
Fall Launch for youth ministry will be the week of Sept 10, 11, and 13. 
Calendar for 2023-2024 is available on each parish's website, under the Youth Ministry tab. 

COST OF REGISTRATION
Cost of registration will be $65 for 1 teen, and $130 for 2 or more teens. You'll be prompted with a link for payment after registering. (If cost is an issue, please let us know, as this is not a problem.)
