We would appreciate receiving a newly completed questionnaire from all dogs.

After submitting this questionnaire, we can provide you with a pre-completed copy for future editing. Not sure which dogs need updating? Enter your preferred email address and we'll send you a list of the dogs we need: https://forms.gle/okwLMaBF4KWDK46B8

By completing this health questionnaire you will help the genetic studies on the health of the Leonberger breed which takes place at the University of Bern and the University of Minnesota, in collaboration with the University of Helsinki, Uppsala University, the Utrecht University, and the Broad Institute.

Please note that the data is collected for research only and your information will be treated confidentially within the involved research centers and scientific collaborators.

It is very important to the success of the Leonberger health studies that we have complete and accurate information on all dogs.
To us, a dog is only as old as their last health update.
Please complete this questionnaire even if your dog is no longer alive, so we can provide a complete medical history.

We accept medical records for all Leonbergers, but we are particularly interested in updates on dogs who have DNA with us (or can provide a blood sample) with the following criteria:

-> Diagnoses of:
  • Cancer (especially osteosarcoma and hemangiosarcoma)
  • Cataracts
  • Glaucoma
  • Infertility
  • Laryngeal paralysis/polyneuropathy (LPN)
  • Sudden cardiac death

-> Dogs that have lived to 8 years or more and have NOT had these conditions.
