All students are expected to report to school daily, however, circumstances will arise which prevent attendance.

This form is used to report your child's absence to school officials.

Your child will be marked absent by teachers AND you will get an automated phone call informing you of your child's absence. This form does not automatically excuse your child(s) absence. If you wish to excuse the absence(s), please submit the proper documentation to the main office upon returning to school.

This form can be used in lieu of a phone call report to the school office.
