FALL SESSION IS FULL FOR SEPTEMBER 2025. BY FILLING OUT THIS REGISTRATION FORM FOR LEARNING TREE, YOUR CHILD WILL BE PUT ON A WAITING LIST AND WE WILL NOTIFY YOU IF ANY OPENINGS BECOME AVAILABLE.

This is for children ages 3-5 years old (as of June 1, 2024). We will explore the alphabet, numbers, shapes and colors through play, music and activities  Classes will be held on  Tuesdays and Thursdays 9:30 -11:00 am at the Pioneer Park Community Resource Center, 610 W. 300 S. Provo, UT.

Due to liability issues, parents/guardians must remain in the building during the class. Parents and siblings of the 3-5 year old will be able register for and participate in other classes at the Community Center.

Learning Tree Release and Waiver of Liability:       This Release and Waiver of Liability is executed in favor of Provo City (the "City") with respect to the MyHometown Initiative (known as "MyHometown") .I desire my child to participate in a preschool class that will be offered at the MyHometown's Community Resource Center located at 610 West 300 South, Provo. I affirm that participation in the activities or services provided by MyHometown's volunteers is voluntary, and acknowledge that there are risks in such participation that cannot be eliminated even when the greatest care is taken. I assume full responsibility for any and all injuries or damages which may occur to me or my dependent as a result of such risks associated with such activities or services provided by MyHometown.I hereby agree to release, indemnify and hold harmless the City, the Church of Jesus Christ of Latter-day Saints (the "Church"), the City's employees, sponsors and volunteers from any liability, loss, cost or expense (including attorney fees, medical and ambulance costs, etc.) suffered by me or my dependents, without limitation. I acknowledge and agree that this is a full, complete and final release of the City and the Church for any harm suffered associated with the tutoring program and my or my dependent's participation.I give permission to the City to use my or my dependent's photograph to publicize MyHometown programs and services.  I agree:
