We understand that our scheduling system may not work best for everyone and we are happy to try and help make scheduling your appointment a pleasant experience.

Participation in this survey is voluntary and will in no way affect if you qualify for WIC services. Please know that if you choose not to provide your contact information, we may not be able to help you resolve your concern as effectively. The vouchers that you are issued for WIC generally expire on the same day of the month. This day is usually the day of the month that you need to schedule your appointment.
