We are committed to removing financial barriers to accessing personal health supplies. Please fill out this Google Form and indicate what supplies you need. Your bags will be ready 2-3 business days (Monday through Friday) after you fill out the form. We will send you an email when your bag is ready and leave it outside the door of our office (Main Building S-180). Your name will NOT be on the bag. To preserve your privacy, we will put the last four digits of your 999#.
