The personal information collected and stored in this form shall only be used for DILG-Baguio City Field Office reporting purposes specifically to the Compliance with submission of KALINISAN. This Office shall not disclose the respondent's (data subject) personal information unless otherwise required by the existing policies and laws, and shall retain this information for a maximum of two-year retention period starting from the processing of the information.
