To best support your school in test planning, you are encouraged to complete this form no later than February 23, 2024. For students who enroll after the statewide testing window begins on March 4th, you are encouraged to complete this within two weeks of enrollment. This form is only valid for the 2023-2024 school-year. Parents and guardians are required to submit and opt-out form annually should they wish to be exempted from MN statewide testing.

MNCA will accept either the online version of MDE's form (below) or the printable form located on the MDE website. Please mail in the form to 1350 Energy Lane, Suite 230 - St. Paul, MN 55108 (ATTN: Mallorie Damchik).

**Families who complete this form will still be assigned testing dates and times.**

** MDE requires all families to complete an online or paper version of the refusal form. MDE also allows for schools to require additional steps to refuse testing. MN Connections Academy may require additional steps from Parents/Guardians after completing this form. **
