By writing my name below and submitting this form, I confirm my commitment to participate in the Mentoring Future Professionals Program.

I understand that if I am selected as a scholarship award recipient, the Foundation will provide the following: MSHA Convention registration at the Student Rate; two nights paid Convention housing within the room block of the Convention hotel; one light meal and a schedule of available meeting times and events with my assigned professional mentor. 

I understand that upon being selected as a scholarship recipient, I agree to meet the entire requirements related to the program, including:

        To register for the annual MSHA Convention (to be held in April) using the Student Attendee rate, and to submit my Convention registration for reimbursement to the Foundation.

        To book a hotel room (if needed) within the  Convention designated hotel block, and to submit my hotel registration prior to the start of my stay for reimbursement to the Foundation.

        To submit a photo of myself to the Foundation for acknowledgement and public recognition of my participation in the program.

        To arrive at the MSHA Convention no later than 9:00 am on the designated day.

        To attend Awards Ceremony with my mentor.

        To attend specific Convention sessions (to be announced later).

        To attend one scheduled light meal (lunch or dinner as determined by the coordinators) during Convention with my mentor and the other student award recipient/mentor(s).

        To complete a written evaluation form at the close of the Convention.

        To complete a short article (approximately 300 words) describing personal Convention and /or MFFP experience along with a photograph of myself for submission to the association’s newsletter/social media.

        All other costs, including parking and meals, will be my responsibility and cannot be reimbursed.
