This survey is administered by Niagara Reproductive Justice (NRJ), a Niagara-based organization whose goal is to provide our community with access to the information and services that empowers them to make their own reproductive decisions. The purpose of this survey is to gather information about abortion access and experiences around abortion in the Niagara Region in order to better understand how we can help people safely access abortion and abortion-related services.
The survey will guide you through 40 questions that ask for details about where and when you accessed an abortion, resources you utilized, the referral process, costs associated with the abortion, the support you received throughout the process, and your overall experiences. ALL QUESTIONS ARE OPTIONAL. If you have had more than one abortion, you may fill out the survey multiple times by exiting out of the browser after submitting your first response and re-clicking on the survey link. Please prioritize your health and comfort while you are taking the survey. If at any point during the survey you no longer wish to be taking the survey, you may stop and your responses will not be collected.
By completing this survey, you agree to have your responses collected and reviewed by members of Niagara Reproductive Justice. All responses to this form will remain anonymous unless you choose to disclose your contact information. Google Forms does not reveal the email address you use to access this survey and your email address is not viewable to the administrators of the survey or anybody else. All questions are optional; please only answer the questions you feel comfortable answering. If at the end of the survey you have questions, concerns, or suggestions, there will be a place to provide your contact information (if you choose) so somebody from NRJ can get in touch with you.
