Washington Hybrid/All Remote Change Request Form
To allow for optimal continuity of instruction and the time needed for administrators and teachers to make the arrangements to effectively serve the student, a parent/guardian must use this form to request a transition from in-person or hybrid learning to full-time remote learning or vice versa. Please note: transitioning a student to or from remote instruction may result in a change in teaching staff and/or instructional staff support, class schedule, classroom composition and social distancing, personal protective equipment and protective classroom equipment, and student transportation services.


***Transition from Full-Time Remote to Hybrid
A parent/guardian who wishes a student to transition from full-time remote learning to hybrid, in-person instruction must submit a request using this online form at least two weeks prior to the requested start date of hybrid learning.
Request must be made by November 23 in order to start at the beginning of the second
Trimester on December 7.



***Transition from Hybrid Learning to Full-time Remote
A parent/guardian who wishes a student to transition from hybrid instruction, in-person to full-time remote learning must submit a request using this online form at least two days prior to the requested start date of full-time remote learning so that teachers can be notified and schedules can be modified.
