I, the undersigned, acknowledge that I have voluntarily chosen to participate in the above-named event. I understand and agree that my participation in this event carries inherent risks, including but not limited to physical injury, property damage, and other unforeseen dangers.

I hereby release, waive, discharge, and hold harmless CANOE & Lakewood Police Department, its officers, employees, volunteers, sponsors, affiliates, and agents from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may occur as a result of my participation in this event, whether caused by negligence or otherwise.
I affirm that I am physically and mentally capable of participating in this event and have no medical conditions that would prevent my safe participation. I understand that I am solely responsible for my own and my family party's health and safety during the event.

I further agree to abide by all rules and instructions set forth by the event organizers, staff, and venue personnel. I understand that failure to comply may result in my removal from the event.

I understand that photography & cinematography will be taking place throughout this event. I understand that small businesses will do their best to not include our faces, but this may not always be possible due to the size of the event. By filling out this registration, I agree and consent to photography & cinematography to the aforementioned & am happy to support Colorado small businesses.
