If your student will be arriving late to school, please sign them in at the front office.

If this is for multiple day input the beginning date on the first date line, then the ending date on the second date line.

If you have a question/concern regarding your students attendance, please call 623-445-3007. This form is for reporting FULL DAY ABSENCES only and messages left not related to reporting a FULL DAY ABSENCE will not be addressed.

Parents/guardians have 24 hours to excuse and absence.
