I hereby agree and declare that I am the legal parent/guardian of the above-named child(ren), that I have carefully read and understand the scope of the Legends Camp LLC activities and I consent to the participation of my child/children to these activities. I understand that activities of the kind described may result in physical injury to my child but nonetheless specifically request that he/she/they be allowed to participate in those activities.

If my child/children require(s) any emergency medical treatment or procedures during the activities, I hereby consent to activity supervisor(s) to make any decision and take any action to arrange for such procedures or treatments at the discretion of the activity supervisor(s).

By signing this Waiver and Release of Liability, with full appreciation of the risk involved, on my own behalf and on behalf of my child(ren), I hereby voluntarily release and forever discharge Legends Camp LLC, its trustees, officers, employees, agents, insurers and contractors from any and all legal or financial responsibility for any and all claims, damages, losses or expenses of whatever kind or nature which my child/children may have or acquire arising out of or resulting from, directly or indirectly, his/her/their participation in Legends Camp LLC. I agree, for myself and my child(ren), not to make any type of legal or equitable claim on Legends Camp LLC, or any of its trustees, officers, employees, agents, insurers or contractors with respect to any injury I or my child(ren) may suffer, whether or not it arises through the negligence, omission, default or other action of anyone affiliated with Legends, including other campers. I hereby release and discharge Legends Camp LLC from any and all claims of any nature whatsoever, which may arise out of the decision to provide emergency medical care, dental care or hospitalization during the activity. I have read the above agreement, understand and accept its terms and consequences, and freely, voluntarily and knowingly sign this waiver. I further agree that if any such claim is made, I will indemnify and defend Legends with respect to any such claim, injury or damage.
