Board Policy-Use of School Facilities #1340 will set the guidelines for this agreement. 

I agree that with the use of this key fob and facility being requested come the responsibility to leave facility in its original condition and turn off all lights. 

Any damages to the school or gymnasium that occur will be the responsibility of the person or group making this request. All costs involved in the repair will be paid by the person or group making this request. Burwell Public Schools will not be liable for any personal injury or theft that occurs during the duration of this agreement. 

School facilities may not be used for activities which would be of lewd, immoral, or an obscene nature or activities that violate state or federal law. The school is a drug free zone-no alcohol or tobacco on school grounds. 

It is my understanding that any regular or rescheduled school event will take precedence over this agreement. 

By completing this request, I understand the guidelines that have been set for using the school facilities and take full responsibility in making sure all guidelines are met.
