III. QX MEMBER AGREEMENT & CLASSROOM POLICY
To use the QUESTABOX Services, I will need to register on the site, pay any applicable fees, and obtain account, username and password. I will not transfer your account information to another person and I will not use anyone else's account(s), username(s) or password(s) at any time without the permission of the account holder.
By enrolling my child with QUESTABOX, I agree to release, indemnify and hold harmless on behalf of myself, my heirs, and representative, QUESTABOX and it's employees from any cause of action, claims, or demands of any nature whatsoever, including, but not limited to, a claim of negligence, which I, my heirs, and representative may now have or will have in the future against QUESTABOX, on account of any personal injury, property damage or accident of any kind arising out of, or in any way related to my child's enrollment in the program. I expressly acknowledge that there are certain dangers, risks, illnesses and personal injuries inherent in participating in online or/and offline programs, which may result from unavoidable accidents or injuries, athletic activities, the use of any equipment, exercise, or other activities or from my or my minor child’s (children’s) physical condition.
QUESTABOX is using Zoom, Google doc, and other educational, creating, or searching apps and websites (e.g., Podcast, Animation, Youtube, Open Library, etc) for classes and projects.
Permission is hereby given for images and/or videos to be taken of my child and used for promotional, website, social media materials through Questabox. Photographed or videotaped children will not be identified by full name in our publications or on our website.

LATE TUITION PAYMENT POLICY: The Client agrees to pay the amount owed before the first class starts. Payment after that date will incur the class delay(s) or a late fee ($5) per week on the outstanding amount.
NO MAKE-UP & NO REFUND POLICY: No make up and No refund for the absent online group classes.
1:1 CLASS CANCELLATION POLICY: If a student is unable to attend a class he or she booked, our cancellation policy will be applied. 1) A student can cancel a class 24 hours before the start of the scheduled class without penalty. 2) If the student cancels it within 24 hours of the start time, the student will be charged a fee ($10). 3) If a student misses it without canceling, the class will be counted as absence. The class is considered as taken (100% class fee will be applied). No make up / No refund for the absence without cancelling.
