Mapping Music for Wellbeing in Sheffield
Thank you for your interest and participation in this project. This survey will take approximately 5-10 minutes.

We are interested in the ways your organisation is using music for health and wellbeing, in any way, no matter how big or small.  For example, if you are a charity, this could be activities that take place for your beneficiaries. If you are a private or public organisation this could be part of your core services or part of an outreach programme, or musical activities for the benefit of your staff. �If you are an individual, or a music professional, this could be provision of music related services.

Who can take part?
Anyone who is employing music or is benefitting from music for health and wellbeing in a professional context in Sheffield is invited to participate in this research. As such we are interested to hear how music is integrated into services, training, resources, facilities, or working practices in such a way that may contribute to the health and wellbeing of service users. In some cases, professionals may support their own health and wellbeing through music.

The main inclusion criteria are:
 - Be located in Sheffield or direct surroundings
 - Working with people to support mental, social or physical health or wellbeing

What will it ask me?
The survey will ask you to explain in brief the relevant professional context, including whether and how you use music and with what objectives, challenges that you encounter and any policy implications that your experiences may have. Additionally, there will be a few questions about yourself and your organisation, and some statements that we would like to know your opinion about.

What is the survey for?
This information will be used to create a map of activities and initiatives for uses of music for health and wellbeing in Sheffield.

How will the data be used and stored?
All data that you will provide will be treated confidentially. Research data will be saved anonymously and pseudonyms will be used to refer to organisations and people in disseminations of the research. General trends in the research data will be analysed and no specific person or organisation will be singled out. The exception to this anonymous treatment of the data is the information that you explicitly have indicated to be happy to share publicly. This data is separately used to create a map of activities and initiatives in Sheffield for uses of music for health and wellbeing. The anonymous research data will be used for disseminations such as publications, presentations and website reports.
