Meal Request Form December 8th Pick Up
Grant Public Schools will be providing meals for students choosing the remote learning option. This week's bag will include 5 breakfast meals and 4 lunch meals per student (half day December 11th). Please complete a separate form for each of your GPS students. This form will close at NOON on Friday, December 4th. **Food allergy: A doctor's note and paperwork will need to be on file with the district before any component of a meal can be substituted. ****If you are an in-person student returning on December 9th DO NOT ORDER REMOTE MEALS THROUGH THIS FORM. Please use the Meal Request Form for December 7th-8th to request food for these two days.
Meals MUST be picked up on Tuesday, December 8th ONLY between the hour of 8:30 am - 11:30 am in the Middle School South parking lot/8th grade doors.
