The COVID Care Map is an open database and software tools to support health system capacity (providers, supplies, ventilators, beds, meds, etc.) The goal is to provide useful tools for health systems adapting and responding to effectively care for the rapidly growing number of COVID-19 patients. We are gathering the best publicly available data that we can find, but we know that it is not 100% up-to-date, complete, or accurate. We need your help to improve the database. This form is to report updates, correct errors, and suggest missing data where you know or suspect an existing data point is wrong or missing.

If you have updates for multiple facilities, please submit the form once for each facility.
