Re Registration for Group Training Booking: Term 1, 2016
This form is for existing clients who have been previously registered and wish to continue training in Term 1 2016. This is to ensure that clients and Monkey See, Monkey Do Fitness are up to date with contact details, medical information, injuries or fitness goals.

Term 1 is upon us!

Re registration is really important for us all, you update me on any changes in your lives that are relevant to your participation in training sessions and I get to deliver a service that best suits you.

Remember, share the Monkey fitness and tell your friends about Monkey See Monkey Do Fitness, don't be selfish! You just might become that friend who solved a problem for them! If the person you refer books for a term of sessions or a series of 5 PT/Small Group PT, you will be rewarded with a $30 voucher to be spent on any classes, PT sessions or a 30 minute massage from Emshia Massage Therapy.

Family Fitness or Kathryn Montgomery the instructor, or anyone acting on her behalf, the facility or any persons involved with this program and testing procedures. *
I understand that I am not obligated to perform nor participate in any activity that I do not wish to do, and that it is my right to refuse such participation at any time during my training sessions. I understand that should I feel light-headed, faint, dizzy, nauseated, or experience pain or discomfort, I am to stop the activity and inform my Personal Trainer. I give Kathryn Montgomery and the staff at the facilities at which we train, permission to seek emergency medical services for me should I become injured or ill with the understanding that I am responsible for any expenses incurred. *
I understand that I am wholly responsible for the safety & welfare of my children while training with Monkey See, Monkey Do! Family Fitness/Kathryn Montgomery or any person acting for her. I understand that Kathryn Montgomery or any person acting for her, is in no way responsible for my children during my attendance & participation. *

MAKING IT HAPPEN: Has your family's schedule changed? Not able to make your usual Group Training Session? Do we not currently cater for your family?
Monkey See Monkey Do! Fitness is all about helping you, fit fitness into your family's life. So if your schedules have changed or your family has changed, I would love to hear from you and am confident that together we can find a soloution. I consider myself a good problem solver, finding ways around the hurdles of family life and tailoring programs to suit a very wide range of clients, that, and lifting very heavy things!

If you would like to talk to me about the hurdles that you face when it comes to fitness, please do! I would love to help you find some ways in which we can make it happen and improve you and your families health, for life. I truly believe that where there is a will, there is a way.

Personal Training, Family Training, Small Group PT, Mum's Groups, Dad's Groups, Community Organisations and Events, School Programs and Workplace Training. Pick a fitness goal/event for the whole family and I can help you get there. Make the most of the better weather during Summer and Spring and book a session in your garden or local park.

For many years I set up and ran circus and arts programs in some of Europe's toughest post conflict situations, Belfast, Bosnia, Serbia, Croatia. If I can help to find soloutions to small problems in those situations, surely I can help you out in lovely Melbourne!
