SF-Annual Easter Egg Hunt: All Ability Accommodations
RE Therapy Services has teamed up with Spanish Fork & Salem Chamber of Commerce to make the annual Easter Egg Hunt accessible for everyone in our community. Please fill out all the information so we can be prepared to best serve you. If you require accommodations for multiple children, please fill out the form separately for each child.

Event Information:
Saturday April 20, 2019

Spanish Fork Sports Park (295 W. Volunteer Drive, Spanish Fork)

For children who register, accommodations include:

1. One parent may accompany a child with disabilities onto the field. Parents who register to accompany their child will be given a badge to indicate they are allowed on the field.
2. Children can participate in a different age group to match their developmental age/abilities. Children who participate in an age group other than their own will need to be accompanied by an adult.
3. A sensory calm down tent will be provided including weighted blankets, visual sensory bottles, tactile bins, and noise cancelling headphones.

Families who register for accommodations will be given a packet including badges for parents and children and information about the event. To decrease wait times at the event, packets can be picked up early at RE Therapy Services (727 South Main Street, Spanish Fork) on April 19th during business hours (8:30am-5:30pm). Packets can also be picked up at the accommodations registration table on the day of the event.
