2025-2026 Medfield Afterschool Program Registration Grades 1-6
Please complete this form to register your child for MAP.  MAP is open on early release days, some non-school days, and during the February & April Vacation weeks. Please complete one form per child.  *Please note that there is a different form for enrolling a Jump Starter, a Kindergartener, and for re-enrolling.  

The registration deadline is Friday, March 7, 2025.   Current MAP families have enrollment priority.  Enrollment at MAP is subject to space availability. To complete the registration process, please mail MAP a $50.00 registration fee ($30.00 for each additional sibling) by the deadline:  Friday, March 7, 2025. MAP holds the registration fee until we are able to offer you enrollment in the program.  Registration is incomplete until MAP receives your registration fee check.   Current MAP families will be billed their registration fee with their 3/14 tuition billing statement.  Checks should be payable to MAP and mailed to P.O. Box 18 Medfield, MA 02052. Please write your child's grade and name in the memo section of your check.  Please type your name below as your signature.  Thank  you! *
