This form provides a way to anonymously report incidents in the Lean prover community.

Please see the Code of Conduct (https://www.contributor-covenant.org/version/2/0/code_of_conduct/) if you are unsure about whether an incident should be reported. When in doubt, please report! We are devoted to building a kind and open community and want no incidents to be overlooked.

This form can be accessed by the code of conduct team. They will do their best to act on the information appropriately. If you do not want this information shared with other maintainers or discussed with others involved in the incident, please let us know, and we will respect your wishes.

If your report is urgent, we also urge you to consider a message on Zulip, since that may be seen more quickly than a report here.
