United 304 Parent Survey
Now that the State of Illinois has moved to Phase 4 and the Illinois State Board of Education has released it's guidance for the re-opening of schools, we at United are seeking your input based on that guidance. Included in the guidance are the requirements that all people (teachers and students) must wear masks in the buildings, temperature check and symptom screenings are performed daily, and preferred social distancing of 6ft adhered to as much as possible. This data will be used to help finalize our plans. Please note that as state and ISBE guidance change, so too may our plans for re-opening. No plans have been finalized at United as of right now. Information about the guidance may be found at
We are collecting email addresses as part of this survey so that we can reach out to individuals as necessary.

If the district chooses to start school in a hybrid option (in-person part-time/remote part-time), would you choose all remote instead?
With face masks being mandatory for in person instruction, how do you intend to respond
Send my child(ren) to school
Keep my child(ren) at home for remote learning.
How do you feel about sending your child to in person instruction to start the year (hybrid or in full)?
I'm totally comfortable with them going back to school
I'm more in favor of them going than not, but I still worry.
I'm not sure I would send them - will wait for data closer to start
I'm extremely nervous about sending my child(ren) to school and would not today.
Given the mandates required to host students in person (masks, 6 ft social distancing, daily health screenings and temp checks), which option below would you prefer as of today?
in person instruction with the above requirements
Total remote learning - no students in the building
Hybrid - tentatively in school 2 days per week and remote 3 days per week to meet social distancing guidelines.
If in person instruction is offered via social distancing guidelines (masks included) AND knowing that we can put as many as 50 people on a bus, how would your student get to school
I would send them on the bus
I will transport my student(s)
In regards to remote learning, the district would plan to send home a Chromebook with every student. Would that gesture solve any/all of your technology needs for remote learning?
