Liability Waiver and Release
By signing this agreement, I acknowledge and agree to the following:
Assumption of Risk: I understand that participation in any activity at Catalyst Community Center involves inherent risks, including but not limited to physical injury and property damage and I voluntarily assume all risks associated with participation.
Release of Liability: I hereby release, waive, discharge, and hold harmless The Catalyst Community Center, its employees, agents, contractors, and affiliates from any and all liability, claims, or demands for personal injury, death, or property damage that may occur during or as a result of my participation.
Indemnification: I agree to indemnify and hold harmless Catalyst Community Center from any claims, damages, or expenses arising out of my participation.
Medical Care: I consent to receive medical treatment in the event of injury or illness during the activity and agree to assume all costs associated with such treatment.
Damage:
By signing this agreement, the renter agrees to pay for any damage to the rented equipment/property that occurs during the rental period, excluding normal wear and tear.
No Drugs or Alcohol Policy
The renter agrees to abide by a strict no drugs or alcohol policy during the rental period. The use, possession, or distribution of illegal drugs, controlled substances, or alcohol is strictly prohibited on the premises. The renter is fully responsible for ensuring that all individuals attending the event or utilizing the rental space comply with this policy. Any violation of this policy may result in immediate termination of the rental agreement and potential legal action.
