Please complete the following referral/application form to express interest in enrollment to a Specialized Option opportunity (Pathfinder School of Innovation, TAPP, Liberty High, or The Phoenix School of Discovery) for the 2024-2025 school year. Applications DO NOT carry over from year to year.
Prior to completing the form, please review the “Specialized Options Overviews” section of this referral for details on each school. Some schools require you to visit their website to complete the application and this information is linked into this form. Once this referral has been submitted, JCPS personnel will contact you regarding placement in a Specialized Option.
