In consideration for being permitted to participate in dance classes and related activities (the "Activities") provided by Enchanted Stage Performance Group (the "Instructor"), I, the undersigned participant, and my heirs, executors, and administrators, hereby agree to the following:

Assumption of Risk: I understand that participating in dance activities involves inherent risks, including but not limited to falls, muscle strains, sprains, broken bones, and other injuries. I voluntarily and knowingly assume all risks associated with my participation, both known and unknown, even if arising from the negligence of the Instructor or others.

Release of Liability: I hereby release, waive, discharge, and covenant not to sue the Instructor, its owners, employees, agents, and representatives from any and all claims, liabilities, demands, actions, and causes of action whatsoever, whether in law or in equity, arising from or related to any loss, damage, or injury, including death, that may be sustained by me or my property, while participating in the Activities.

Medical Acknowledgment: I certify that I am in good physical condition and have no medical condition that would prevent me from safely participating in the Activities. I agree to notify the Instructor of any medical conditions or physical limitations.

I have read and fully understand this Waiver and Release of Liability. I voluntarily sign it, intending to be legally bound by its terms.
