2017-2018 Capt. John L. Chapin Orchestra Fundraising Contract
Parents and Students must read, understand, and sign this contract.


For the Student:

____________________________________ understand that by participating in organized booster fundraisers, am responsible for all monies and items. Lost items must be reported immediately.  Lost/stolen monies and/or items will be my responsibility.  Unpaid items will be returned in as new condition for full credit. Damaged items will be my responsibility at item cost.  
(*Candy bars deemed unsellable, will not be considered for full credit, therefore any returned candy bars will be inspected for credit consideration).  

By agreeing to participate in booster fundraisers, I adhere to the following guidelines:
1) I agree to represent the orchestra and booster organization appropriately and respect the selling guidelines/locations of each fundraiser.  
2) I agree to be responsible for all items/monies associated with the assigned fundraiser.
3) I agree to complete all sales and return monies/items by the posted deadline of each assigned fundraiser.
4) I understand that I will not be allowed to participate in following fundraisers until items/monies are cleared.  
5) I understand that outstanding fundraiser monies/items will be reported to the school business office and may result in holding of records for registration/withdrawal purposes and will also result in loss of fundraising privileges and funds.
