Hello and thank you for taking time for this survey. This survey is part of a comprehensive review of the mathematics program in grades 6-12 by Looney Math Consulting which has been contracted to conduct the review. This survey is designed to gather information about perceptions you have about math in general and gather some of your perceptions about learning math. This survey is designed to be anonymous. Information about gender and grade level of your students are optional, but will help us look for trends.
