BE SURE TO ENTER A WORKING PARENTAL / GUARDIAN EMAIL SO TO SHARE THE INFORMATION WITH PARENTS . THEY WILL RECEIVE A COMPLETED EMAIL UPON SUBMISSION.

Every student athlete who participates on a SOSD High School team (Hockey, Rugby, Lacrosse, Football, Soccer) must complete (once a school year) the enclosed Concussion Education module as designed on the Parachute Concussion Smart Awareness & Education Program.  Completion and submission of the module will identify the risks involved with high impact, contact sports and an understanding of the role  Madison Ridgen Athletic Therapy  (MRAT) will play in our sports seasons.  This module has been created by the SOSD and data of completion will be recorded by each collegiate.
