Baselining & Goal Setting Questionnaire
All information received on this form will be treated as strictly confidential. The purpose of this questionnaire is to get a better understanding of where you currently are in regards to your fitness, find out what you like to do, find out your goals and find out what has worked and what hasn't worked for you in regards to meeting your goals in the past. You should expect to spend a minimum of 15 minutes to answer the questions. I know that seems like a lot of time, but the more mindful and present you are in doing this, and the more you think through your answers, the more likely you'll reach your goals.
