I waive all claims and liabilities against COLLIN’S® for any injury sustained or illness suffered by the above named Child as a result of consuming the food or participating in COLLIN’S® activities in relation to the COLLIN’S® Kids Workshop whether
by the negligence of COLLIN’S® or otherwise.
I, the parent/legal guardian of
the participant, consent to my Child participating in the said Workshop.
I hereby confirm that my Child has no known allergy of food and/or any medical condition or issue that may affect my Child’s participation in the said Workshop.
I hereby grant permission to COLLIN’S® to take photographs and/or videos of me
and my Child for any commercial purposes. I acknowledge that COLLIN’S® owns all the rights to the photographs and/or video and may use the same for publishing, printing and/or advertising and any other commercial purposes.
I hereby waive any right to inspect or approve the use of the photographs and/or videos and any right to royalties and/or compensation arising from or related to the use of the same.
I hereby release, discharge, and indemnify COLLIN’S® and any COLLIN’S® appointed 3rd party for the said Workshop, against any claim and liability including but not limited to claims of negligence, defamation, invasion of privacy, rights of publicity, copyright infringement by or on behalf of my Child as a result of my Child’s participation in the said
Workshop.
I hereby agree to indemnify and hold COLLIN’S® free from any claims as set out above
and indemnify COLLIN’S® for any reasonable legal fees, costs and expenses incurred by COLLIN’S® with respect to defending any claims made against COLLIN’S®.
Any reference to COLLIN’S® above shall include the business entity that is operating the COLLIN’S® restaurant, i.e. Iron Chef F&B Pte Ltd and its employees, directors, officers, partners and volunteers.
