Burning Man Remembrance Plaque

The Remembrance Plaque will be displayed at the Man Base.
Please fill in as many or as few of the optional fields as you would like.
Submissions must be received by July 20th to be included on this year's plaque.
Submissions after the 20th will be added to next year's plaque.

With the maximum number of elements, the plaque listing of names will appear like this:
