Midwest Witches Bazaar Sponsor Form 2021
Hello. Thank you for sponsoring the Midwest Witches Ball, which in turn sponsors the Michigan Witches Ball 2021, by Detroit Conjure LLC. This event allows us to continue to support the charities that do so much to help families in Michigan. Sponsorship comes with the perk of a table at the Midwest Witches Bazaar, as space permits.

This table can be used for fundraising, vending, or information, counseling services, or product demonstrations. We are practicing a more spaced floor plan in order to encourage better distancing.

We ask that all sponsors abide by Health Department/Governmental guidelines and regulations. Proper handwashing, use of hand sanitizer, masks if able, and cleaning wipes for your displays are part of our sponsor advisory.

This event abides by all applicable statutes, ordinances, and executive orders. We also abide by all rules and requirements of our venue. This includes the prohibition on smoking inside the venue. This incudes e-cigarettes and vape pens, hookahs, as well as water pipes. We also ask that you not burn incense inside the venue. If you choose to violate these protocols, you will assume all damages and liabilities.

Please be advised, if you have debt of any kind to the Michigan Witches Ball, you will be unable to sponsor this event until all are paid in full. There will be no exceptions to this.

If using my space to vendor, I understand that I am solely responsible for collection of all sales taxes. *
I understand that I am responsible for all set up and clean up for my area. This is to begin no earlier than one hour before the opening hour of the event for set up, and end no later than 30 minutes after closing for breakdown. All parties and goods shall be transported off premises by myself or my organization no later than 30 minutes after the event's closing. The event will not provide table covers, stays, tape, or other display equipment. I understand that my space will be preselected by the event, with no guarantees of placement. *
I agree that the event and its organizational sponsors reserve the right to refuse the donations of any, and all, sponsors/organizations. I also understand that all items and content must be suitable for viewing of those under the age of 18 years old because this is a family oriented event. I agree that once registered, payment is via Paypal ref: PayPal.me/BlackWolfRunning and is due immediately upon completion of this form in order to be registered. Sponsorship must be paid in full. Deadline is Sept 15 , 2021. All payments should reference the event in notation line. Checking yes serves as your electronic signature. We will issue no refunds unless in the event of an act of God, or Gods, or civil disorder which causes the inability to conduct the event. In the event of a pandemic, event will abide by all directives from civil authority. In the event of a state shutdown of venue, event will be rescheduled, Partial refunds will be given no later than 10 business days after event date if requested within that time. *
I agree that I will be allowed assistants for this event. The event staff WILL NOT be on hand to assist loading or unloading. Minor children who assist at the event are my own responsibility. I agree that no refund will be issued in the event, if as a sponsor, I do not attend this venue after having made my payment. There is no exception for absence based on illness, suspicion of infection, or quarantine status. I agree that if I fail to provide ads by Sept 15, 2020, in a pdf or jpg format, that my will not be in program book. In the event that we reject your sponsor registration, all funds will be returned to you. Checking yes serves as your electronic signature. If donating a sponsorship, I agree that no refund will be given in event of sponsor's scheduling conflict, or other personal issues. *
I understand that self promotion is key to success of this event. I understand that with advertising, I greatly increase my space's attendance and visibility at this event. I understand that the event makes no promises, nor guarantees, on sales or attendance numbers. *
By checking yes on this form, I agree that I will not hold the facilitating organization of this event, or its agents, liable for any injury or loss incurred in association with this event in any way. I understand that I am solely responsible for my own safety, goods, and business practices. I affirm that I have all necessary licenses, permits, and permissions to engage in any sales, services, or disbursements. This agreement also is to be held as applicable to any agents operating on behalf of myself or my company/organization. I affirm that I have the right to make this agreement. I understand that spacing of tables must not be changed. I will not move my table within 3 feet of another sponsors table, as that undermines social distancing. For that cause, all spaces will be left as they are in placement. Violation of this rule could lead to my direction to move my display back to its place, regardless of time of event remaining. *
Unfortunately, emotional support animals are not allowed at this venue. *
This event has a No Drama/No Diva policy. Any violations will lead to a single warning. Repeated violators will be removed from the event. I understand that it is in the interest of the community to bring all concerns only to event staff so that they may ensure a safe and orderly resolution of any conflicts. If I cause a public disturbance I will be asked to leave. If I refuse to leave the premises due to disruptive/aggressive/threatening behavior, I understand that police may be called, and the venue may have me trespassed. *
Waiver: event and staff reserve the right to refuse any sponsor application, and should this occur the donation will be refunded. The sponsor shall defend, save and hold harmless the event, associated organizations, their respective officers, agents, board members, staff, volunteers, sponsors and assigns from any claims, damages, losses, liability or expense which may arise, and shall not be held responsible for any loss or damage due to fire, accident, theft, weather, acts of God (s), pandemic, vandalism or any other loss or injury whatsoever or not specifically described herein, whether past, present or future. Sponsors and exhibitors must make provisions for safeguarding their goods. Sponsors and exhibitors assume full liability for protecting, care and maintenance of exhibitor’s property. ANY VENDOR/SPONSOR OR EXHIBITOR NOT HOLDING VALID LIABILITY INSURANCE EXHIBITS AND PARTICIPATES AT THEIR OWN RISK AND ASSUMES ALL LIABILITY. I understand that at events such as these it is very often Extremely Loud. In order to facilitate the most easy transactions for myself and fellow sponsors/vendors, I agree that I will not engage in screaming, hollering, yelling, or otherwise barking throughout the venue regarding sales or goods. This can be quite disturbing and alarming to both the patrons as well as other sponsors/vendors. *

The laws of the state of Michigan shall govern the validity, construction and effect of this Agreement. All lawsuits arising out of this agreement, wherever derived, shall be resolved in Macomb County in the state of Michigan. *
I agree to keep confidential, and not to disclose to third parties, any information provided by the event and staff, pursuant to or learned by me or my organization, during the course of this sponsorship regarding the details of my sponsorship unless I have received the prior written consent of the event to make such disclosure. This provision shall survive expiration and termination of this event. *
I agree that in the event that either party is unable to perform its obligations under this Agreement as a result of a force majeure, neither party shall be liable to the other for direct or consequential damages resulting from lack of performance. *
