USFS Fire Management System Learning Questionnaire for Agency Administrators
As identified in the 2016 Life First engagements sessions, we have committed to conduct learning reviews of successful operations across a spectrum of complexity in each Forest Service Region to improve decisions this fire season and beyond, as well as to begin developing a capacity for organizational learning to ensure information is distilled into actionable knowledge and effectively communicated. Our goal with the questionnaire is to help meet these commitments, and to identify the strengths and weaknesses of the wildland fire system to learn how all the components are operating, and share best practices.

We request that these questionnaires be completed on all type 1 and 2 incidents with USFS jurisdiction, and welcome responses on any others.

The Agency Administrator (AA) questionnaire asks for the home unit name, and the Incident Management Team (IMT) questionnaire asks for the incident name. Though this information can identify specific AAs and IMTs, this information will not be used in any reports or published anywhere. It is however necessary to align contrasting questions and ensure both questionnaires are referring to the same incident.
