Gavilan STEM Lending Library Request Form - Fall 2025

The Gavilan College STEM Center offers a material lending program for STEM majors, student tutors, STEM Academy members, MESA students, and non-STEM major students enrolled eligible STEM courses.

***This request form also serves as your "Contract of Responsibility"***
By completing this form, you accept responsibility for the available requested material(s) upon loan.

-Materials and tech equipment are limited and are for lending to registered Gavilan students based on need.
-This form will not guarantee the availability of the item(s) requested. If you are NOT eligible for requested items, you will receive an email from Marissa Herrera before your scheduled pick up date.
-The STEM Lending Library is for eligible students who are in good standing with the college, do not currently have holds on their student account, and/or have not failed to return previously loaned materials.

I will return all borrowed materials and associated components upon request of a Gavilan STEM staff or faculty member (contacted via email or phone) on an agreed upon date

I understand that if I do not return all materials by the agreed upon date given by Gavilan Staff or Faculty, a HOLD will be placed on my student account/records and I will lose any opportunity for future use

I will be responsible for any lost materials or accessories as well as any partial or complete damage to the material or accessories

I understand that if I fail to return the materials and all its parts I will be responsible for replacing it at market value
