I understand that participation in the Lehi Youth Football Camp involves physical activity and the risk of injury. I hereby give permission for my child to participate in the camp and release Lehi High School Football, its staff, coaches, volunteers, and affiliates from any liability for injuries or damages that may occur during the course of normal camp activities.

I certify that my child is physically able to participate and I will inform organizers of any medical conditions that may affect their participation.

I give permission for my child to be photographed or recorded during the Lehi Youth Football Camp for use in team promotions, such as social media posts, flyers, or the school website. I understand that no names or personal information will be shared without additional consent.
