2012 Los Altos Robotics Scrimmage Application Form

Required fields are indicated with a small red asterisk.*

Use this form to apply for the Los Altos Robotics Scrimmage at Blach School on Sunday, October 21, 2012. Our capacity is limited to 24 teams. If we are oversubscribed, preference will be given to teams in the Los Altos / Mountain View area that applied early and have confirmed a volunteer to help with the scrimmage. Registrations will be accepted until September 30 or until all slots have been filled. IF YOU NEED TO MAKE CHANGES AFTER YOU SUBMIT YOUR REQUEST, JUST SUBMIT A NEW REQUEST.

There will be a $20 registration fee for the scrimmage, to cover expenses of running the Los Altos Robotics organization and events. Payment details will be emailed to the team coach and manager once they are available.

NOTE: Participating in the Scrimmage DOES NOT guarantee your team a slot in the Los Altos Robotics Tournament on November 18. Registration for this Tournament (and all others in Northern California) is handled by Playing at Learning. Watch www.norcalfll.org carefully for instructions. In particular, BE SURE TO PAY YOUR REGISTRATION FEES AND SUBMIT
YOUR PAPERWORK ON TIME in order to have the best selection of tournament dates and venues for your team.

The information you supply here is for the purposes of administrating the LosAltosRobotics events. This information will be freely shared among the volunteers running those events. A summary of the teams with team names, coaches names, school and grade information is generated from this information and shared publicly. This summary does not have phone numbers or email addresses. We will take reasonable precautions not to spread this information improperly. If you have any concerns about this, please note them in the "other information we should have about your team" section at the end of the registration form.

One to two days after you submit this form, you will receive an email from Linda Ren, Registrar and Volunteer Coordinator, confirming your application. She will send follow up emails if your team's status changes and to confirm your volunteer assignment. Feel free to contact her at any time. She can also help you with problems submitting this form. She can be reached at lindaren@gmail.com or (650) 450-0546.
