フォーム後半には、登録料 $120（返金不可） のお引き落とし、ならびにご入学後の 教材費・授業料のお引き落としのための ACH 銀行口座登録 の項目がございます。
銀行口座の登録は、保護されたサイト DocuSign に移動して行っていただきます。
DocuSign の手続きが完了しましたら、 再度こちらのフォームにお戻りいただき、最後までご記入のうえ Submit を押してください。
送信後、Google Form よりご登録のメールアドレス宛に確認メールが自動送信されます。
なお、ご入学が確定した方には、2月末日までに ご入学手続きおよび準備に関するご案内をお送りいたします。

Dear Parents,Thank you very much for choosing BCA Saturday School.
In the latter part of the form, you will find sections for the $120 registration fee (non-refundable), as well as ACH bank account registration for future withdrawals of tuition and instructional materials fees after enrollment.
The bank account registration will be completed on the secure DocuSign website.
Once you have completed the DocuSign process, please return to this form, complete all remaining sections, and click Submit.
After submission, a confirmation email will be automatically sent to the email address you registered via Google Forms.
Please note that students whose enrollment is confirmed will receive further information regarding enrollment procedures and preparation by the end of February.
