2020-2021 Return to School Instructional Model Change Request Form
Warren Township Board of Education Policy #1648.02 - Remote Learning Options for Families outlines procedures for a parent to request to transition from in-person services to full-time remote learning or vice versa.

A parent may submit a request to change models by notifying the school principal at least 30 days in advance of the desired change. Requests at the beginning of the school year or at the beginning of a new phase of the return to school process will be evaluated at those times. Requests for students to change models at any other time may be granted should conditions and resources allow.

This form will serve as a written notification to the school principal.

Elementary Parents - The final opportunity make a change request for the 2020-2021 school year is approaching. Requests must be submitted by the end of the day on Thursday, March 18, 2021. These changes will go into effect on April 12, 2021 for the remainder of the school year.

Middle School Parents - All parents will be asked to determine an instructional model for fourth marking period in a targeted survey on Wednesday and Thursday, March 17 and 18. Those changes will go into effect on April 12, 2021 for the remainder of the school year.
