- A Program of Work will be requested after you submit this form. This is a mapping of program participation across the academic year.
- A roster with all active student's STARS numbers will be reqested after you submit this form.
- At the end of a year, a request for information surrounding your students' involvement in community service, professional development, dual credits or certifications will be sent out for funding support from NMPED to the program.
