Parent/Student Intake and Scheduling Form
Liberty Local School District and Leopard Cyber Academy invites you to register yourself and your child or children to attend an intake and scheduling meeting with the staff of the cyber academy. The intake and scheduling meeting is the first step for attending the online school. The purpose of our intake and scheduling meeting is to provide background knowledge of the online cyber school, increase understanding of policies and procedures of the online program, parent and student expectations for a successful educational experience, and the scheduling of classes for students.
Once you register to attend the meeting your spot(s) will be reserved and you will receive written notification from the staff of Leopard Cyber Academy of the date, time(s) and place for your meeting.
If you and your child or children cannot attend on the date, time(s) we provide in our follow-up message to you, please contact Leopard Cyber Academy staff so we may reschedule your appointment at 330-759-0807.

Parent or Guardian - Please provide a phone number where you can be reached.
Parent or Guardian - Please provide your address including street, street number, and city.
Parent or Guardian - Please provide a valid email address if you have one.
Parent and Guardian - Please provide the name or names of the children you wish to attend Leopard Cyber Academy. Include the child's age and the grade each child will be entering this coming school year.
